Good Faith Estimate Notice

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You have the right to ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.  Make sure to save a copy or picture of you Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms/gov/nosurprises.

Insurance Panels

I am a provider on a few insurance panels. If I am a provider on your panel, I will bill your insurance panel directly for reimbursement. You may have a copay/co-insurance due at each session which I will collect at the beginning of each session. Sometimes you may also have a deductible that must be met before your insurance panel will pay any portion of the sessions. It is your responsibility to gain clarity about your coverage before the commencement of therapy.

If I am not a provider for your insurance panel, at your request, I can provide you with a monthly statement to submit to your insurance panel for reimbursement. You would need to check with your insurance panel to determine what portion of the session might be reimbursed.
